The sanctuary of the Mare de Déu de Loreto is an 18th-century Baroque temple in the upper part of Ulldemolins, which replaced an earlier 16th-century chapel built to venerate an image left behind by a pilgrim from Reus. The current building was designed in 1758 and construction, led by master builder Pere Sans, began in 1762. It has a single nave without side chapels, a barrel vault with lunettes and a sacristy behind the main altar; the façade opens with three access arches, an oculus beneath the bell tower, two side windows and a rose window.

The 1770 Baroque altarpiece was lost in 1936. In 1985 the façade was decorated with sgraffito, and in 1991 the interior was painted with Eastern Orthodox-style murals depicting the history of salvation, the work of Chilean artist Juan Etxenique i Celis. The sanctuary is protected as a local cultural interest site (IPAC identifier 11397). Every 10 December the Majorales group holds a ceremony in honour of the temple's patron.
